Please click on the link to download an activity registration form if paying with check or cash. Forms need to be completed, signed, and mailed or delivered to the Recreation Department main office located at Mt. Hood Golf Course.

VOLUNTEER OPPORTUNITIES
All volunteers over the age of 18 are required to:
1.    Complete a CORI Form once per year
2.    Provide a clear photo copy of their Drivers License (Will not accept faxed copies)
3.    Complete a Coaching Application Form (If applicable)
